1. Health History

Please fill out the Health History form outlining any existing health conditions, surgeries, or major illnesses in the past and what you’d currently like to work on.

2. Unique Data

Please also fill out the ‘Unique Data’ prior to your session. This information enables the Bioresonance machine to complete your 7000 item Wellness Scan and perform the vital electromagnetic feedback. Rest assured, your details are safe, private and confidential.

 
 

points to note

Please bring any medications or supplements you are currently taking so we can check their potencies and suitability.

There are no print outs given from scans, however you are welcome to take notes during your appointment, so please bring paper and pen.
